Fort Dearborn Life

  • Term Life Insurance $15,000, $25,000 or $50,000
  • Short Term Disability (1st day accident, 8th day illness/ 26 weeks of benefits), $150, $250, or $300 per week benefit

Vision Insurance (Summary of Benefits)

  • The IOGAWV vision plan provides 1 exam per 12 month period; 1 pair of lenses per 12 month period; 1 pair of frames per 24 month period; 1 pair of contact lenses per 12 month period when provided in lieu of frames and lenses.
